GEMOXIA02: HEPATIC ARTERIAL INFUSION OF GEMCITABINE-OXALIPLATIN FOR SECOND-LINE THERAPY IN NON-METASTATIC UNRESECTABLE INTRA-HEPATIC CHOLANGIOCARCINOMA: A MULTICENTRIC SINGLE-ARM PHASE II STUDY

This study is looking at a new treatment approach for people with bile duct cancer inside the liver (intra-hepatic cholangiocarcinoma). When this cancer cannot be removed with surgery and standard first-choice treatments stop working, new options are urgently needed. In this trial, doctors use a method called hepatic arterial infusion. Instead of giving chemotherapy through a general drip into a vein, they deliver two chemotherapy medicines (gemcitabine and oxaliplatin) directly into the main blood vessel feeding the liver. Because liver tumours rely heavily on this artery for blood, this method aims to deliver a high dose of medicine straight to the cancer while sparing the rest of the body from strong side effects. The main goal is to find out if this treatment shrinks tumours after four months. Researchers will also monitor safety, patients' daily quality of life, and whether shrinking the cancer makes future surgery possible.

Sobre o que é este estudo?

Intra-hepatic cholangiocarcinoma is a type of cancer that starts in the small bile ducts located inside the liver. In many patients, the cancer cannot be safely removed with surgery because of where it is positioned or how close it is to important blood vessels. When the first course of standard chemotherapy stops keeping the cancer under control, doctors look for effective second-line treatments to help manage the disease.

This trial investigates a specialised technique called hepatic arterial infusion (HAI). The liver is unique because it receives blood from two main sources. While healthy liver tissue gets most of its blood from one vessel, liver tumours mostly feed on blood from another vessel called the hepatic artery. By inserting a thin tube into this artery, doctors can deliver two cancer-fighting medicines—gemcitabine and oxaliplatin—directly to the tumour. This targeted approach aims to attack the cancer cells with higher concentrations of medicine while reducing general side effects across the body.

Researchers want to see how well this treatment works over four months. The main measure of success is whether the tumours shrink or disappear on scans. The study team will also monitor how long the treatment keeps the cancer stable, how it affects patients' day-to-day well-being, and whether the shrinkage might even allow an experienced liver surgeon to safely operate on tumours that were once considered inoperable.

Pontos principais

  • Tests targeted chemotherapy delivered directly into the liver's main blood supply.
  • Designed for adults with bile duct cancer inside the liver that cannot be removed by surgery.
  • Aimed at patients whose first round of standard cancer treatment has stopped working.
  • The primary aim is to check whether tumours shrink after four months of treatment.
  • Researchers will also track quality of life, side effects, and whether surgery becomes possible.

Quem pode ser elegível?

This study is open to adults aged 18 and over who have been diagnosed with bile duct cancer that started inside the liver. To be considered, the tumour must be inoperable (meaning surgeons cannot safely cut it out), but the cancer must not have spread to distant areas outside the liver.

The trial is specifically for people looking for a 'second-line' therapy. This means you must have already tried an initial standard cancer treatment that has either stopped working or is no longer controlling the tumour.

Your clinical team will also review your overall health, kidney and liver function, and general fitness. You will need to be well enough to undergo the procedure to place the delivery tube and to manage regular hospital visits for treatment and monitoring.

Este estudo pode ser adequado para você?

Responda a estas perguntas rápidas para ver se você pode ser elegível. Este é apenas um guia — a decisão final é da equipe de pesquisa.

  1. I am aged 18 or older.
  2. I have been diagnosed with bile duct cancer inside the liver (intra-hepatic cholangiocarcinoma).
  3. My cancer cannot be removed with surgery, but it has not spread outside my liver.
  4. I have already received a first-line cancer treatment that is no longer working.
  5. I am well enough to attend regular hospital appointments for tests and treatment.

O que envolve a participação?

Taking part begins with a minor procedure or scan-guided treatment to place a thin tube (catheter) into the hepatic artery that supplies your liver. Once in place, you will attend the hospital regularly to receive the chemotherapy drugs (gemcitabine and oxaliplatin) infused through this tube.

Throughout the study, you will have regular blood tests, physical examinations, and appointments with your care team. You will also be asked to complete short quality-of-life questionnaires about how you are feeling in your daily life. Every few months, you will have imaging scans (such as CT or MRI scans) to check if your tumour is shrinking or staying stable.

Treatment continues for as long as it is helping you and side effects remain manageable. After treatment finishes, the study team will continue to stay in touch through routine follow-up visits to check on your health and recovery.

Riscos e benefícios potenciais

A potential benefit of joining this study is receiving a targeted treatment that could shrink your liver tumour, control symptoms, and potentially make surgery an option in the future. However, there are also risks. Having a catheter placed carries a small chance of bleeding, pain, or infection, and the chemotherapy drugs can cause side effects such as nausea, tiredness, lowered blood counts, or changes in liver function. Taking part is completely voluntary. You are free to leave the trial at any point, and choosing to stop will not affect the quality of your regular NHS care.

Perguntas frequentes

What is hepatic arterial infusion?

It is a procedure where chemotherapy is delivered through a small tube directly into the main artery that feeds blood to the liver, focusing the medicine where it is needed most.

Will I be put on a placebo (dummy treatment)?

No. This is a single-arm study, which means every participant accepted into the trial receives the active study treatment.

Can this treatment cure my cancer?

The main goal is to control the disease and shrink the tumour. In some cases, shrinkage might make it possible for a surgeon to remove the tumour later, but a cure cannot be promised.

Do I have to pay to take part in this trial?

No. In the UK, clinical trial medications and study-related tests are provided free of charge through the hospital.

What happens if the treatment stops working for me?

Your doctor will monitor your scans closely. If the cancer grows or the side effects become too difficult, the study treatment will stop and your team will discuss other supportive care options with you.
