Taking bisoprolol and duloxetine together may cause your blood pressure to drop too low or your heart rate to slow down more than intended. You might feel dizzy, lightheaded, or faint, especially when standing up. Additionally, duloxetine can slightly increase the levels of bisoprolol in your blood, making its effects stronger.

Why this happens
Duloxetine can interfere with a specific liver enzyme (CYP2D6) that helps break down bisoprolol, leading to higher levels of the heart medication in your system. Furthermore, both drugs can lower blood pressure and heart rate through different pathways in the body, leading to an 'additive' effect where the combined impact is stronger than either drug alone.
