Taking diltiazem with apixaban can increase the amount of apixaban in your bloodstream. This may increase your risk of bleeding, such as bruising more easily, nosebleeds, or more serious internal bleeding.

Why this happens
Apixaban is broken down in the body by a specific enzyme (CYP3A4) and a transport protein (P-gp). Diltiazem slows down the activity of both this enzyme and the protein. Because the body cannot clear the apixaban as quickly as usual, the levels of the blood thinner rise.

Back to contentsYou can usually take these medicines together, but your doctor should be aware of the combination. Watch closely for any signs of unusual bleeding or bruising. If you notice blood in your urine, black tarry stools, or a nosebleed that won't stop, seek medical advice immediately. Your doctor may occasionally need to adjust your dose depending on your age, weight, and kidney function.
