Taking orlistat (Xenical) and omega-3 fatty acids (fish oil) together can prevent your body from absorbing the fish oil supplements properly. This means you may not get the full health benefits of the omega-3.

Why this happens

Orlistat works by blocking the enzymes (lipases) in your gut that break down fat. Because omega-3 supplements are fats, orlistat stops them from being broken down and absorbed into your bloodstream. Instead, the oil stays in your gut and is passed out of your body in your stools.

To make sure you absorb the omega-3, you should take your fish oil supplement at least 2 hours before or 2 hours after taking your orlistat dose. Many people find it easiest to take their omega-3 at bedtime if they take orlistat with their main meals.
